Ovarian remnant 2: This patient had also had removal of her uterus, tubes and ovaries. At that surgery, the ovaries were encased in adhesions. Similar to the previous case, shreds of ovarian surface cortex were left behind and later proliferated to form the reddish cystic structure within the circle which was found in the left cul de sac at the bottom of the pelvis. This patient had painful intercourse and painful bowel movements. The rectum is partially visible just to the left of the "c" in "cul-". The end of the vagina would lie just at the top of the circle. The location of this ovarian remnant would be a good explanation for this patient's symptoms.
