Ovarian remnant 1: This patient has had her uterus, tubes and ovaries removed. Dense ovarian adhesions were present during that surgery and removal of the ovaries was difficult. During removal of the left ovary, some of the surface of the left ovary remained stuck to the left pelvic sidewall. Just as if 2 pages of paper were glued together and you tried to peel them apart later, you would find shreds of one page attached to the other and vice versa, so some of the surface of the left ovary remained stuck behind on the left pelvic sidewall. The ovarian tissue can survive, despite the absence of a normal blood supply. The ovarian tissue can be stimulated by pituitary hormones and can proliferate and form cysts and produce normal amounts of estrogen. This patient had cyclic left lower quadrant pain. This surgical photograph shows a cystic structure of the left pelvic sidewall enclosed in the circle. After it was completely excised, the pathology report showed normal ovarian tissue.
